The closest I have ever come to anything like dextrocardia was in my Junior Clerkship Internal Medicine longcase, and actually this was a Right lung atelectasis of the lower zone! Only thing the patient knew was his Cardiac Arrhythmias, and suspected PE (so later that day he was supposed to have a CT Pulmonary Angiography) cz he had a sudden collapse as he was at home, prior to admission. So me I go all in, looking for signs for cardiac disease and Respiratory exam, with him being so stable, and not in any distress! The upset started with the respiratory exam, slight tracheal deviation to the right (just jotted that down), no sounds in the lower zones, then continued with the rest. At the palpation point in cardiovascular exam, that's when I realise this wasn't a "relatively normal patient", PMI on the right - 4th ICS, just on the right boarder of the sternum, with cardiac dullness on the right! I just couldn't believe it! Of course adrenaline is in, asking them if they had previously done a chest X ray and they hadn't, and I knew karma had come for me. Auscultation gave similar results, findings weren't familiar to me by that time! The examiners come, my Senior House Officer and Professor, and I presented everything the way they were! Then with exam findings, they couldn't believe what I told them! Asked if I had touched the patient and the patient said yeah.. my professor palpated the cheat himself, and then called the SHO to feel the PMI yes they got it!! They asked for the steth and got no sounds in the right lower zone! Their clinical picture was clear of course they made their diagnosis, fairly not far from my anticipation..but not a dextrocardia! Me after that? I passed!
