@reverendofdoubt And you can give higher doses than 10 mg IV, because even if the chosen those is above an “analgesic ceiling,” the higher dose will provide longer-lasting pain relief.

A single dose of toradol in the ED for a patient with CKD or who is on AC is fine.

@emergencypdx @HemalPatelDO You’re PPV is going to be higher when PE is on the differential than a case like this used to screen for DVT. If we’re talking POCUS and + dimer, then I’d get an upstairs duplex or repeat in a wk. If we’re talking (-) duplex and + dimer, we’re done. What are your thoughts?

You have a patient with a swollen leg & you strongly suspect DVT. D dimer elevated. Ultrasound negative. They have no chest pain or SOB. Vitals are normal. (no PE signs or sx) What would you do next?

@embasic @MedFactChecks @dr_cellini I disagree even for that value when done as routine for every trauma. That’s the reason there’s many attendings now that rely on the FAST to r/o abdominal injury and consider it a necessary part of all trauma workups.

@j_bartnik Also, looking at the issue as a whole. I don’t have a problem w/ background checks & restricting assault rifle purchases of individuals w/ mental health issues (although this is a slippery slope). Nor do I object to restrictions on convicted violent criminals. Reform is needed.

@DrTonyWeaver Ok. I have data that certainly suggests as gun laws get looser, shootings go up. I have an easy solution - regulate guns, especially military grade assault rifles. What’s your solution for mental health, poverty, education, social determinants?

@j_bartnik There is a place for legislation, but I don’t think it’s limiting guns. There are more important factors mental illness, poverty, lack of education & other social determinants that I believe lead to the use of guns. Guns are not the problem. They are metal, powder. & plastic.

@DrTonyWeaver This report is nonsense. Loose gun laws and gun-related homicides have a nearly linear relationship. Those laws are made on the state level. If you were to step back from current discourse, i think you’d agree that giving assault rifles to anyone without any checks is a bad idea
