Across 885 RCTs, postop pain was driven by RA, not intra-op opioids.
When RA works, opioids add little & ↑side effects (↑PONV).
Take-home: prioritise RA.

This NMA by de Carvalho et al looking at the performance of various commercially available VLs published in @Anaes_Journal a few days ago makes for fun reading. Not sure I agree with the design & reporting entirely, but will share thoughts on this later.
