Saw a patient today with a hemoglobin of 1.9 g/dL. For context, a level that low is almost incompatible with normal consciousness, but she walked right into the clinic on her own feet. For three long years, she lived with crushing weakness and since last 6 months breathlessness from just walking across a room. Why didn’t she get help sooner? At first, it was because the kids had crucial school exams and later her husband was reluctant to deal with the hassle of a hospital admission. Her health was treated as a background inconvenience. When we dug deeper, it got worse. A year ago, her Hb was 6.4 g/dL. A doctor explicitly told them she needed immediate admission. The family refused, walked out with a basic strip of iron tablets, she took them for two weeks, forgot about them, and nobody in the house ever bothered to check on her or remind her. She didn't even come to the hospital today because of the air hunger. She came because her periods had completely stopped for months. Her body was so profoundly starved of iron and oxygen that it literally shut down her reproductive axis just to divert what little blood she had left to her heart and brain. It’s completely heartbreaking. A woman will literally bleed her body dry, gasp for air for years and keep working silently, only to be brought to a doctor when her normal functioning stops. Please check on the women in your homes. Stop letting them normalize chronic exhaustion.

I was forwarded this image on WhatsApp, and it’s been making rounds all week. This was June 1997. Moi was in power. Fuel prices and cost of living were high and our rights were under attack. 29 years later, different era, same struggle. Kenya is blessed with so much potential, we cannot remain trapped in an endless cycle of corruption, poor priorities and injustice. The first time I shared her story a colleague offered to support with two terms fees at Machakos academy at Ksh.50,000 per term. The next I shared, I got support for her Form 2 full year fees. And a commitment for full year pocket money. Last term a friend called and asked about the girls i support. I told them three girls and they committed to cater for one with full year fees. I’m currently exploring support for this girls university or college as I’m assured she’ll go and my commitment to her was 8:4:4 without stopping as long as she stays the track, which she has since class 6 when I took her in. Look at her now. I took her in when she was in class 6. She’s now in Form Four. When I first met her, under that street light, she was destitute, shy, couldn’t speak more than two words, or look you in the eye. But now she’s confident, speaks out, assured, she laughs and smiles a lot. And importantly her grades are great. Ooh God. All on you.

Whoever came up with the phrase cheap is expensive certainly never fueled at Astrol Petrol Station. 1. It is 100% Kenyan, founded in 2000 by the late Thayu Kamal Kabugi and is now run by the son, James Mwangi. 2. They not only own the land their stations are built on to cut costs but also manage their stations directly rather than franchising. 3. Their fuel contains a high 91 PON rating, which is higher than the premium fuels around, including V-Power. 4. They are not only 2 - 7 shillings cheaper but also never hoard fuel whenever there are shortages. We don’t appreciate such companies enough

@alexmwanzo I was insured with @APAInsuranceKE. My car was hit properly from the back. It needed a boot, wing, back door, front door and rear number. It took 5 days for assessment and garage allocation and 25 days for complete repair.

DID YOU KNOW?? The nipple is not just a delivery valve, but a highly sophisticated biological sensor? Through a process often called "RETROGRADE MILK FLOW," the nipple actually "samples" a baby’s saliva to customize the nutritional and immunological profile of the breast milk in Real-time. Let me explain 👇🏾👇🏾 ​1. The "Saliva-Nipple" Communication Loop: ​When a baby latches, a vacuum is created. Research has shown that this vacuum causes some of the baby’s saliva to flow backward into the mother’s mammary glands. For example - ​Pathogen Detection: If the baby is fighting a virus or bacteria, those pathogens are present in their saliva. ​Once the mother's body detects these foreign pathogens through the nipple, her immune system immediately ramps up the production of specific antibodies. ​Within hours, the next "batch" of milk is enriched with those exact antibodies, passing them back to the baby to help them fight the specific illness they are facing. A very efficient system! ​2. Composition Changes by Age and Environment: ​The nipple and breast tissue act as a biological lab, adjusting the milk’s chemistry based on the baby’s immediate needs detected through the baby-mother interaction. A. Fat Content Increases or decreases based on caloric need. Detected via latch frequency and saliva hormones. B. Antibodies Spikes when the baby is sick (Leukocyte surge). Pathogens detected in baby's saliva. C. Water Content Increases during hot weather to prevent dehydration. Detected via baby's hydration markers/saliva viscosity. D. Melatonin Increases at night to help the baby sleep. Happens via the Mother’s Circadian Rythm 3. The "Leukocyte Surge": ​A fascinating study showed that when a baby is ill, the number of white blood cells (leukocytes) in the mother's milk can skyrocket to levels seen only during the very first days of nursing (colostrum). ​The "backwash" of saliva alerts the mammary glands to the infection. ​Leukocytes from the mother’s own bloodstream migrate(infiltrate) into the milk. This effectively turns the milk into a "personalized medicine" tailored to the baby's specific infection at that exact moment. ​4. Beyond Nutrition (The Bio-Feedback Sensor): ​The skin of the nipple and areola also contains Montgomery glands. These small bumps secrete oils that serve multiple purposes: A. ​Olfactory Signalling: The scent of these secretions is similar to amniotic fluid, helping the newborn "find" the nipple. B. ​Bacterial Barrier: The oils have antimicrobial properties to protect the nipple from the very bacteria it is "sampling" from the baby. C. ​PH Regulation: They help maintain a specific skin pH that encourages the baby's healthy oral microbiome. ​This constant, two-way dialogue ensures that breast milk is never a "static" product; it is a dynamic, living fluid that evolves every few hours based on the biological data provided by the baby's saliva. The late Professor Bongani Mayosi (51)🕊️🕊️who left us far too soon on the 27 July 2018. After a courageous two year battle with clinical depression, this brilliant mind and compassionate soul tragically died by suicide. His passing sent shockwaves through the global medical community and left an irreplaceable void in South African academia and healthcare. Professor Mayosi was not just a world renowned cardiologist, academic, and researcher he was a true medical pioneer whose passion and brilliance illuminated the field of heart disease. As Dean of the Faculty of Health Sciences at the University of Cape Town (UCT), he inspired countless students and colleagues with his vision, humility, and unwavering dedication. He is lovingly remembered as a “medical legend,” a title he earned through groundbreaking discoveries that changed lives. His identification of the genetic basis for arrhythmogenic right ventricular dysplasia stands as one of South Africa’s most significant medical advances since the first human heart transplant an achievement that continues to ripple across the world with hope and healing. His remarkable journey was marked by excellence at every step. -BMedSci (Bachelor of Medical Science), University of KwaZulu-Natal (1986) -MBChB (Bachelor of Medicine and Bachelor of Surgery), University of KwaZulu-Natal (1989) -DPhil (Doctor of Philosophy), University of Oxford (2003) -AMP (Advanced Management Program), Harvard Business School (2016) He also held prestigious fellowships and honors that reflect the deep respect he commanded internationally.

Professor Mayosi’s life was a beautiful testament to what one dedicated individual can achieve for humanity. Though his physical presence is gone, his legacy of scientific excellence, mentorship, and quiet strength lives on in every heart he helped heal and every young doctor he inspired. We honor his memory with gratitude, admiration, and a deep sense of loss. 🕊️ May his soul rest in eternal peace, and may his story remind us to care for those fighting invisible battles with the same compassion he showed the world.
