Who needs to be held accountable? With all this, will it be enough to rescue reputations of @MRCP & @RCPhysicians ? Can the college survive this?

This is a disaster for @RCPhysicians Errors can happen but how they respond now will be the litmus test. Why so long to identify the error? What measures to prevent recurrence? How to support and remedy training for all those affected? Investigate and publish if patient harm.

Anaesthetic care (these often with higher risk profile) results in few complications and cheaper. The only reason for not doing it is lack of availability.
