How can carotid artery blowout (CABO) be prevented during re-☢️ for recurrent NPX cancer? In this teaching case, authors describe use of planned carotid artery sacrifice and cerebral revascularization 🔘Pt was 🔼 risk for CABO due to: 360 degree encasement of L ICA, NPX ulceration, 3rd course ☢️ 🔘Pre-RT, balloon test occlusion of L ICA demonstrated feasibility 🔘After re-☢️ (72 Gy/24 fx) ➡️underwent L ICA embolization + low-flow STA-MCA vessel bypass 🔘At 7.9 yrs post RT, pt was free from CABO and tumor control was maintained 🔗 - tinyurl.com/2nn6486m @ASTRO_org #radonc #hncsm

🧠⚡ New data on AVM radiosurgery outcomes! A large single-center retrospective study of 134 patients treated with LINAC-based stereotactic radiosurgery (SRS) for cerebral arteriovenous malformations found: 🩸 Low hemorrhage risk after SRS • 9 post-SRS hemorrhages over 816.6 patient-years • Overall bleeding rate 6.7% • Annual bleeding rate just 1.1% • No hemorrhage-related deaths 🙌 📈 Who is at higher risk? • Higher Spetzler-Martin AVM grade (III–IV) • Prior treatment, especially embolization or surgery + embolization • Age, sex, AVM size, and SRS dose were not associated with bleeding risk ⏳ Important clinical pearl • Hemorrhage can still occur years later — even after angiographic obliteration • Latency period after SRS isn’t fixed → long-term imaging follow-up matters 🔍 Bottom line: LINAC-based SRS offers a relatively low post-treatment hemorrhage risk compared with untreated AVMs and historical series, but careful patient selection and prolonged surveillance remain essential. @ASTRO_org advancesradonc.org/article/S2452-…

Can a smartphone📱replace surface-guided RT systems? 🤔 Surface-guided radiation therapy (SGRT) systems are: 💰 Expensive 🧠 Complex 🌍 Often inaccessible in low-resource settings In this proof-of-concept study, iSGRT, a smartphone-based SGRT system using built-in LiDAR on Apple devices to track patient surface motion in 6 degrees of freedom (translation + rotation), found: ✅ Extremely high correlation with ground truth (r² ≥ 0.995 translations, ≥ 0.975 rotations) ✅ Minimal bias (≤0.9 mm, ≤0.4°) ✅ Accurate tracking across breathing patterns—including irregular breathing ✅ DIBH duration nearly identical to SDX (difference = 0.03 seconds!) ❓Would you find this tech helpful for your practice? advancesradonc.org/article/S2452-… @ASTRO_org
