Does timing matter? Maybe. Does methodology matter? Absolutely! Happy to discuss this elegant SWOG S1404 analysis showing how rigorous statistics can overturn an appealing clinical hypothesis 💥 If you’re interested in melanoma, adjuvant immunotherapy, and why bias matters, I hope you enjoy this week’s #WeekInOncology with @OncoAlert 🧡 @hospitalclinic @myESMO @_SEOM

🕒 Does the time of day of immunotherapy matter? It is NOW time for This #WeekInOncology by @OncoAlert 🚨, this time with our AF colleague @MarioBalsaMD 🇪🇸Discussing the latest in #Melanoma A multicenter analysis of 628 patients with high-risk #melanoma treated with adjuvant #Pembrolizumab found no significant association between infusion timing and recurrence-free or overall survival after accounting for important sources of bias. These findings challenge previous reports suggesting earlier immunotherapy infusions improve outcomes and highlight the importance of rigorous statistical methodology in interpreting timing-related treatment effects. ⏰ Does the timing of immunotherapy really matter? This week's #WeekInOncology by @OncoAlert 🚨 features an outstanding review by our AF colleague @MarioBalsaMD 🇪🇸, who breaks down the latest evidence in #Melanoma with his trademark clarity and clinical insight. 📄 Discussing the JCO OP analysis of the NCI/SWOG S1404 trial: A multicenter study of 628 patients receiving adjuvant #Pembrolizumab found no significant association between infusion timing and recurrence-free or overall survival after rigorous adjustment for time-related biases. 💡 An important reminder that robust methodology is just as critical as the clinical question itself—and that not every appealing hypothesis withstands careful analysis. 📖 JCO OP: ascopubs.org/doi/abs/10.120… 👏 Excellent summary by @MarioBalsaMD!! @EladSharonMD @KenGrossmannMD @jmkirkwood @VernSondakMD @DrSapnaPatel @mihalcioiui #WeekInOncology #OncoAlert #Immunotherapy #SkinCancer #ClinicalResearch

🤔 Long-term survivors after anti–PD-1 in advanced melanoma (n=58)! What about 4-1BB⁺ CD4 T cells? academic.oup.com/jimmunol/artic… ▪️Baseline CD4/CD8 T-cell subsets did not differ by survival 💥 Long-term survivors showed an early increase in 4-1BB⁺ CD4 T cells during treatment 🎯 This was associated with increased LAG3⁺TIM3⁺ CD4 T cells and later reduced FOXP3⁺ CD4 T cells 🩸 On-treatment immune dynamics, rather than baseline, may better identify durable responders @OncoAlert @OncoReporte @myESMO @_SEOM

Perioperative EV plus pembrolizumab is now approved for every MIBC patient headed for cystectomy, cisplatin-eligible or not. The FDA expanded the label today on the strength of a head-to-head win over cisplatin-based chemo, the standard we've used for 20 years. 🧪 KEYNOTE-B15/EV-304: 808 cisplatin-eligible patients, perioperative EV + pembro vs neoadjuvant gemcitabine + cisplatin 🎯 EFS: HR 0.53 (median not reached vs 48.5 mo), p<0.0001 📈 OS: HR 0.65, p=0.0029 This is the first time a perioperative regimen has beaten cisplatin-based chemotherapy on survival in eligible MIBC. Reviewed by FDA through Project Orbis, cleared five weeks early. 👉bit.ly/evpembro-mibc @tompowles1 #BladderCancer #MIBC #GUonc

🤰 Pregnancy and risk of melanoma: a matched retrospective cohort study (854,193 pregnant vs 854,193 nonpregnant women) ovid.com/jnls/melanomar… 🤔 Pregnancy-associated melanoma has long been debated, with hypotheses that gestational immune tolerance and elevated sex hormones may promote melanocytic transformation 🎯 No increased invasive melanoma risk: HR 0.92 (CI: 0.78–1.07) 🎯 No increased melanoma in situ risk: HR 1.01 (CI: 0.82–1.24) 🔍 Sensitivity analysis remained consistent Gestation may reshape immunity, but here it does not seem to gestate extra melanoma risk 🧬 @OncoAlert @OncoReporte @_SEOM @myESMO

Some trials deserve more than just reading the abstract! That’s exactly what #WeekInOncology is all about💥 @OncoAlert LITESPARK-022 is a perfect example of why expert discussion matters: balancing DFS benefit, toxicity, and the unanswered OS question! Congrats @nataliagandur! Always setting the bar high 🧡

This #WeekInOncology by @OncoAlert| GU Edition 🚨 Today, our GU Faculty Dr. Natalia Gandur 🇦🇷 reviews the LITESPARK-022 trial, led by OncoAlert Faculty member Dr. Toni K. Choueiri.🇺🇸 📄 Adjuvant Pembrolizumab plus Belzutifan for Renal-Cell Carcinoma NEJM, July 1, 2026 LINK👉 buff.ly/OqaCA64 The phase 3 trial showed that adding the HIF-2α inhibitor belzutifan to adjuvant pembrolizumab after nephrectomy significantly improved disease-free survival in high-risk clear-cell RCC. 🔬 Key results: ➡️ DFS benefit: HR 0.72 (95% CI, 0.59–0.87; P<0.001) ➡️ 24-month DFS: 80.7% vs 73.7% ➡️ No significant OS difference at interim analysis ➡️ Higher grade ≥3 toxicities with combination therapy (52.1% vs 30.2%) 🎥 Watch Dr. Gandur’s expert discussion on this important advance in GU oncology. #KidneyCancer #GUOncology #RenalCellCarcinoma #Oncology #Immunotherapy #OncoAlert @DrChoueiri @tompowles1 @apolo_andrea @neerajaiims @montypal @Silke_Gillessen @AOmlin @brian_rini @PGrivasMDPhD @TiansterZhang @AlbigesL @cdanicas @amerseburger @sonpavde @DrDanielHeng @declangmurphy @seanmmcbride @HHammersMD @shilpaonc @DrRanaMcKay @scserendipity1 @DrYukselUrun @scocmem @ravikanesvaran @AmandaNizamMD @drenriquegrande @BraunMDPhD @EfstathiouEleni @nataliagandur @crisbergerot @motzermd @PBarataMD @DrIacovelli
