After 20 yrs in EP, I’ve never seen hype like PFA. Point-by-point RF—when done properly—delivers excellent results, with no clear superiority of PFA. Are we driven by better outcomes, or by speed, simplicity, and industry momentum? Time to refocus on science over marketing.

Those strokes are quite serious--> one due to air embolism related to sheath. 5 clearly related to the procedure. The trial was paused and mitigation was made--no more high-risk pts (CHADSVASC > 5 excluded). After mitigation, they then had no more strokes, but in just 74 pts. Most of us ablation docs have gone years without seeing a stroke. That there were 6 speaks to a real problem with this technology. What's more there were 6 deaths in the trial 5 PFA 1 AAD. One death in the PFA group occurred at 45 days when exercise -> VF-> death. Autopsy showed no obv MI and it was called CV but unrelated to PFA. Was it? ahajournals.org/doi/10.1161/CI…
