@markiewicz_md @jcgeorgemd @mmamas1973 @JayMathewsMD There are anecdotal reports of response to VKA when DOAC failed. Would give it a try if the patients symptoms aren’t too bad at the moment. But would monitor closely.

@markiewicz_md @jcgeorgemd @mmamas1973 @JayMathewsMD 3 months of warfarin with INR 2.5-3. Had a patient with failed DOAC with non resolving LAA thrombus, completely resolved with warfarin. DOAC may be good for preventing, but not best for resolving organized thrombus in few.
