For IM lengthener, I prefer to use 2 Schanz pins as a marker of rotation to avoid inducing any unintended rotational mal-alignment. For the proximal pin, I use guide wire from the 6.5 cannulated system, then drill over it, then apply by hand. I will go as posterior as possible.
