@mdonnino @bradloncar This is all true - and it points to something else that we should be doing, especially in the era of AI and big data (and rare side effects), which is much bigger and better post marketing surveillance. I'd rather something that works gets out there in the wild.

Fair enough. I hear your points and do see that seems to be the approach often taken. That said, the example of the COVID vaccine story is a little nuanced as we actually had 3 to begin with and the 3rd (J and J) was taken off the market for the comparative analysis to the other two (not placebo). Specifically, J&J had a pause for risk:benefit analysis in April of 2021. At that time the analysis that was performed was a risk:benefit comparing J&J vaccine to "no vaccine" (ie., the placebo analysis). The result favored J&J as the side effect was very rare and therefore the recommendation was to keep on the market. The data then did not change between April and December 2021 when they did a relook. In the relook, the analysis changed to a comparison of J&J versus mRNA vaccines. The subsequent analysis concluded favorably for J&J for placebo but not when compared to mRNA. The decision to compare J and J to mRNA instead of placebo shifted the risk:benefit leading to removal of J&J from the market with the exception of cases where someone could not receive mRNA (initially) and then ultimately it just came off altogether I think because that was not sustainable. Anyhow, I do agree with your points and now do recognize a bunch are going to market as you describe but I also do think safety analyses generally include risk:benefit that generally would seem to include alternative options. At least in the case of JJ, the comparison to other vaccines resulted in FDA removing from the market.

I stand by this comment from last fall. Moderna and others need to take their messaging to the general public. There’s not going to much public outrage about yesterday’s flu news because there’s been no meaningful public education on mRNA by our industry.

@mdonnino @bradloncar after all, we had two covid vaccines, each based on its own placebo controlled trial, and we never had a comparison. This was (correctly) viewed as a good thing.

@mdonnino @bradloncar Controversial take: choice of comparator should be a risk for the sponsor (Moderna) to take. It should not be an FDA mandated item. FDA establishes safety and efficacy, with data in the label. More meds in the same space = more choice, lower prices.

@nycexpatmom You should know that early on they went on ventilators because the met the guidelines for it (spO2). By may it was apparent that the guidances didn’t apply. Every good (level 1) hospital updated their practices by then.
