@KeeganMatheson FYI both Raleigh and Polanco had better splits from the right side of the plate this season. The decision for Little is mind boggling

There was even a fine argument for Jeff Hoffman in the 8th. The #BlueJays were 2-1 and the guy who hit 60 homers is leading off the inning. The most important inning of the ball game.

@KeeganMatheson The whole “turn Raleigh and Polanco around” is such BS. Guess what… they are still really good hitters from the right side. You had your no. 1 and 2 relievers available. You use them. You live and die by them. Using your 7th reliever in the highest leverage spot is unforgivable

I didn’t expect to see Little over Domínguez in this inning and it didn’t work. Stunning turn here for the #BlueJays

@gforbes It seems they may have aggregated FFS and non-FFS rates together. Which if you look through some threads you’ll see we have discussed it at length and I appreciate the insight from physician colleagues on it.

@SabraGibbens @HilaryWollis @TomPark1n @alandrummond2 This has been a massive issue in my area (the Kawarthas). I believe all family physicians are the AAP model, and they are really struggling financially. We have lost many in a short period. Making my life harder in practice (not that it’s all about me 😉)

@mkcavy @HilaryWollis @TomPark1n @alandrummond2 Having an NP on my team is great for me and my patients, but adds $$$ to my already very high overhead costs. It’s not a sustainable business model. I don’t blame new grads for avoiding this path. If I had known then what I know now, I would’ve chosen differently. 4/4

Ford says Shoppers Drug Mart will be “held accountable” for head office push on pharmacists to make — and bill OHIP for — unnecessary prescription checks But how could SDM be draining $1.4M/wk from OHIP without notice? Why is SDM paid twice what an MD gets for a check?

NEW: Premier Doug Ford responds to @CBCToronto reporting on Shoppers Drug Mart pressuring pharmacies to hit MedsCheck quotas "If you're telling the pharmacist to do MedChecks which they don't have to do...then they're in deep trouble" CBC story: cbc.ca/news/canada/to… Video👇

@mkcavy @HilaryWollis @TomPark1n @alandrummond2 The patient enrolled model was created in part to discourage the one issue per visit hassle. I see fewer patients per day so that I can address multiple complex issues in one longer visit. The downside is that I cannot see 30 people per day. My wait time is often > 5 weeks. 2/n

@KCGraham6 That’s so unfortunate and really is a disservice to the program. We have, for lack of a better term, “rostered” patients that are of greatest need - the are either on compliance packs or our appointment based program. This is our focus beyond discharge

@mkcavy From my perspective in Primary Care, MedsCheck post hospital discharge is high yield, I think scheduled checks with CKD would also be high yield. Unfortunately I can confirm the absolute onslaught of low yield checks the last few years.

Who wants to know what work goes in to, what I consider, a good, quality #MedsChecks? Almost like an AMA session. Ask away, and I’ll share how we perform them at our group of pharmacies.

@KCGraham6 All in all, took me around 60 minutes. But, in the end, the patients medication regime is far better than it would have been had we just followed the discharge Rx

@mkcavy @PharmacistMama Although I ageee with your assessment, the destruction of US community pharmacy happened when the first pharmacy signed a contract with AWP -1% 30 yrs ago. very profitable contract, but PBMs boiled the frog slowly down to AWP-18%. If AWP = 20% GP, margins razor thin now.
