It was my pleasure to take part in CTO Essential in Istanbul. Chairing a session and performing a live case of complex RCA CTO with excellent team. Great meeting and education with stellar faculty as always. Credit to @ogoktekin for a fantastic effort as always.

Such cases are difficult to decide and judged on case by case basis. FFR would tell u if angina is because of it (doubtful). It however needs to be corrected now to reduce long term risk of ST and problems. This remains our Achilles in PCI, undersizing. The saving grace is how good the current DES r, not every undersized or under expanded stent will cause a problem but almost invariably every time we have ISR or occlusion it’s undersized or under expanded.

@ColletCarlos @mmamas1973 We have a floating 3.5 mm stent in a 5.5 mm proximal LAD, that has been there since 2021, in a patient with severe effort angina since then, MLA 8.7 mmsq, 3.1x3.4 MLD artery diameter 5.5mm

F 65, full metal jacket of the LAD, classical limiting angina on walking , the cath report states that the ostial LAD stent was 3.5 mm, the mid LAD stent was 3/28, the distal was 2.75, here is the angio #leftRadialFirst today, your thoughts please, poll in thread

@evandrofilhobr @JulioBarbiero Great work Evandro. Testament to the skills and perseverance and shows that doing CTO when clinically needed does matter. The number of times I had patients telling me that they can’t believe what they can after CTO PCI and exercise etc is phenomenal. QoL matters above all.

I’ve seen several testimonials like this in my career but I have to confess that they always move us and push us forward. After a complex 3rd-attempt CTO PCI in Chapecó-SC with my friend @JulioBarbiero, the best reward: seeing our patient walking, jogging, doing Pilates… and running into his granddaughter’s arms. Clinical judgment, skill & teamwork is all that matters and allow us to reach our patient’s expectations. (Patient consented and allowed the video share)
