Women are very particular about their looks, and assume that most men who approach them, especially when they (the women) look good, is romantically interested in them
So approaching to point out a supposed flaw in that appearance instead, while bearing a straight, uninterested face, will get their attention and elicit concern.
The effectiveness is less about the line itself and more about the quick sequence it creates, where a moment of mischief and mild embarrassment is followed by the contrast between the deadpan expression that sells the setup and the playful face that follows after the joke is revealed.
