A new development in global health and development. As of today, the US has officially withdrawn from the World Health Organization, the United Nations' health agency. The withdrawal was announced a year ago by an executive order, so this has been in the works for quite some time. The cited reasons have mostly to do with the COVID episode a few years ago and how it was handled by the WHO back then with the lockdowns, masks, vaccines, and so on and so forth. One way to summarize it is as a disagreement between a principal and an agent. Like the World Bank and the IMF, the WHO is a creation of the post-World War II international order. Global branding aside, it was always primarily an American organization, being motivated by US funding to the greatest extent. The WHO is losing about 25% of its staff according to media reports. There was a joint resolution of the US Congress in 1948 that brought the US into the WHO. It requires that the US pay around $260 million to $278 million in membership fees or dues for the 2024–2025 period before its exit can be completed. For it's part, the US has signaled that it will certainly not be paying those fees. Obviously, from a practical standpoint, the WHO cannot force a sovereign nation like the US to write it a check. According to media reports, the WHO Executive Board is meeting next month to discuss all this because to the WHO, these unpaid arrears create a legal and diplomatic "limbo" regarding whether the U.S. has actually "left" or not. What happens next? Well, when it meets, the Board will probably decide: do we remove the US from the member list today? Or do we keep them on the books as a "delinquent member" to presrve the legal claim to that $260M? That is, if the US ever wants to rejoin (e.g. under a Democrat), the WHO would then make paying the fees a condition of rejoining. Who knows. My bet is that neither the U.S. nor the WHO is likely to back down, though. In the end, the US obviously has the most leverage. Perhaps the WHO should just let it go and has little choice in the end. In some ways, fighting this is like clinging to the old world, which, whatever happens, is probably not coming back. Although it is increasing its contributions, China is not likely to assume the WHO role traditionally occupied by the US. Leading the WHO doesn't seem to really fit within their policy frameworks, which are mostly national development plans and international commercial arrangements. It seems very safe to say that if it was going to, it would've announced it in the last year. In a time where much of the world is looking more inwardlly, events like these speak to the urgency of developing countries to transcend ethnic biases and develop themselves as quickly as humanly possible. As for the future of global health and development, its next evolution likely lies in the private sector. Today is a transformative moment for international health, as it's moving away from the post-WWII centralized model toward a more fragmented, privatized landscape.
