Calling all ID pros. Seeking advice for mgmt of 40y/o AML pt s/p HSCT w/persistent VRE BSI w/out 🔍 nidus, WBC 0.1 ANC 0. Persistently +BCx on d14 dapto 10mg/kg, CPT 600mg q8h, & 5d synergy gent. Plan on switching to LZD. Any guidance? @IDPharmacometrx @mwupce @TheIDApprentice
