Clinical Case Series challenges of metabolic effect. A 40-year-old woman underwent Roux-en-Y gastric bypass one year ago for morbid obesity. She presented with fatigue, muscle cramps, and tingling in her hands and feet. She stopped taking vitamin supplements several months ago and has not attended medical checkups. On physical examination, she was pale, with a smooth, shiny tongue, dry skin, and slow reflexes. She did not present any edema Laboratory studies show hemoglobin of 9.8 g/dL mean corpuscular volume of 104 fL, vitamin B12 at 120 pg/mL total calcium at 7.8 mg/dL vitamin D at 12 ng/mL PTH is 100 pg/mL.
