@chaiiwithkaran You can. If you don't have access to USG. I used to. Landmarks are fairly easy and consistent for ulnar N specifically. ❌ ❌ ❌ Don't inject directly in the groove though,it might cause compartment syndrome like features and lead to persistent nerve injury/damage.

BLOCKS WORK! Conducting uneventful cases oneself gives joy but getting such messages from students years after they have graduated is absolute bliss! This is from someone who is working in one of the most remote tribal areas of Odisha. This is a PNS guided block. #Anaesthesia

@chaiiwithkaran Look for little finger flexion extension,for ulnar N the issue is that it is in the corner pocket under the subclavian A,which is difficult to localise without USG. Try supplementing with ulnar N block at elbow for below elbow procedures,or at axilla for above elbow.

@nehapadhi What I have experienced is even with finger flexion and extension Ulnar often gets spared.

@chaiiwithkaran Send surgeon out for tea while giving any block. Be patient and always wait till you get the appropriate response before injecting the drug (Finger flexion-extension for the above case for example),don't inject above 0.5 mA or below 0.3mA current.

@AnjuGup40734644 Exactly ma'am! Honestly I had no idea about Anaesthesia till I actually joined my own residency. I have realised we shouldn't diss online platforms indiscriminately,it all depends on how one uses them.

@nehapadhi One anesthesia JR told me APFEL scoring accurately on day 1 of joining. When asked how he knows, he told he was taught in Marrow

I love books,haven't used any of these online platforms or even coaching for PG entrance but recently took a MBBS class and was pleasantly surprised by how much the students knew about a "short" subject like Anaesthesia,have to appreciate online platforms for that! #MedTwitter

@DrAkhilX @darkknight11010 Dr Akhil,this is not rabies. Laryngeal spasms might sound like a bark but nothing like this. Also a 4 month long course of the disease seems highly implausible. This seems like power of suggestion induced behaviour in the child,he requires counselling.

No proper train service from Nuapada/Kalahandi/Balangir districts that one can travel sambalpur at early morning and return at late evening. Kindly take this matter seriously. Daily commuters, students, patients, employees are hugely suffering. @DRMSambalpur @EastCoastRail
