@martinvars It would be really interesting to know what your tech team approach is as a new company. For new software, the impact of AI is even higher IMO. Also makes me sad that you are not hiring dev remotely or in Spain for what I saw, any reasons for that ?

I spent over a decade building biotech companies. Prelude Fertility. Overture Life. Gameto. Human life is not an abstraction to me. I have seven kids. Three months ago I started building my first AI healthcare company because something fundamental changed. AI crossed a threshold from interesting to transformative. Look at the data. In breast cancer screening, Google's AI reduced false positives by 5.7 percent and false negatives by 9.4 percent compared to radiologists in large US and UK studies. In dermatology, deep learning systems have matched or exceeded board certified dermatologists at identifying malignant skin lesions. In controlled diagnostic reasoning studies published in JAMA, GPT 4 outperformed practicing physicians when both were given the same clinical cases. AI reads brain MRIs in seconds. It does not get tired, distracted, or defensive. It scales instantly. And yet the debate in rich countries is whether AI is good enough to assist elite doctors. Meanwhile four billion people worldwide have no access to a doctor at all. No diagnosis, no prescription, no triage, no second opinion. And this is not only a developing world problem. In the United States, more than half of all counties have no practicing cardiologist. Large parts of rural America have no psychiatrist, no OB GYN, no specialist within reasonable driving distance. These are care deserts inside the richest country on earth. The real comparison is not AI versus the best hospital in Boston or Madrid. It is AI versus nothing in rural Africa, parts of India, Latin America, and also in rural Texas, Wyoming, Mississippi, and dozens of other American counties where specialty care simply does not exist. Perfection is a luxury for wealthy systems. Access is survival for everyone else. For billions of people, and for millions of Americans, AI is not a threat to medicine. It is the first doctor they will ever have. We just closed a 10 million dollar seed round to build this company properly. The official announcement is coming soon. I am more excited about this than anything I have worked on in years.

I meet dozens of AI Health startups every week and can tell you this is a big deal. Most of them will become redundant once this gets adoption. Your medical triaging, nutrition, fitness training, rehab, mental health all in one place.

I've never felt this much behind as a programmer. The profession is being dramatically refactored as the bits contributed by the programmer are increasingly sparse and between. I have a sense that I could be 10X more powerful if I just properly string together what has become available over the last ~year and a failure to claim the boost feels decidedly like skill issue. There's a new programmable layer of abstraction to master (in addition to the usual layers below) involving agents, subagents, their prompts, contexts, memory, modes, permissions, tools, plugins, skills, hooks, MCP, LSP, slash commands, workflows, IDE integrations, and a need to build an all-encompassing mental model for strengths and pitfalls of fundamentally stochastic, fallible, unintelligible and changing entities suddenly intermingled with what used to be good old fashioned engineering. Clearly some powerful alien tool was handed around except it comes with no manual and everyone has to figure out how to hold it and operate it, while the resulting magnitude 9 earthquake is rocking the profession. Roll up your sleeves to not fall behind.

Tesla has been working hard toward shipping Full Self-Driving (Supervised) in Europe for over 12 months now. We have given FSD demos to regulators of almost every EU country. We have requested early access, pilot release programs or exemptions where possible. We have developed & shared detailed safety evidence for FSD, now public in our latest Safety Report. And we have driven over 1 million kilometers safely on EU roads across 17 different countries (internal testing).  Our main path to success is partnering with the Dutch approval authority RDW to gain exemption for the feature. This involves proving compliance with existing regulations (UN-R-171 DCAS) + filing an exemption (EU Article 39) for yet-to-be-regulated behaviors like Level 2 systems off-highway, system-initiated lane changes with hands-off the wheel etc.  Some of these regulations are outdated and rules-based, which makes FSD illegal in its current form. Changing FSD to be compliant with these rules would make it unsafe and unusable in many cases. While we have changed FSD to be maximally compliant where it is logical and reasonable, we won't sacrifice the safety of a proven system or materially deteriorate customer usability.  As a result, we are gathering evidence to get exemptions on a specific rule-by-rule basis. Unfortunately, the real world fleet-proven safety wins alone are considered insufficient.  Currently, RDW has committed to granting Netherlands National approval in February 2026. Upon NL National approval, other EU countries can immediately recognize the exemption and also allow rollout within their country. I did the thing. Switched from 17 Pro Max to iPhone Air. It’s the nicest one this year.
