In the GLUCO-TAVI trial, 100 patients undergoing TAVI were randomized (1:1) to peri-procedural glucocorticoids (IV methylprednisolone + 5-day oral prednisone) vs standard care to evaluate whether inflammation modulation could reduce post-TAVI conduction disturbances requiring pacemaker implantation. At 1 month, permanent pacemaker implantation occurred in 8% of patients receiving glucocorticoids vs 16% in the control group (RR 0.50, 95% CI 0.16–1.55; p=0.23), showing a numerically lower event rate, although the study was not powered to demonstrate statistical significance. At 1 year, no significant differences were observed in PPI (RR 0.67; p=0.41) or new LBBB (RR 1.12; p=0.66). Peri-procedural glucocorticoid therapy was feasible and appeared safe, with no signal for excess complications, mortality, or adverse events. These hypothesis-generating findings suggest that a short course of glucocorticoids represents a potential strategy to reduce post-TAVI conduction disturbances. Evaluation in larger, adequately powered randomized trials is warranted before any conclusions regarding clinical applicability can be drawn. A strong thread ran through #EuroPCR 2026: how to better prevent, anticipate and solve complications in daily practice. “Learning by sharing” remains one of the foundations of interventional cardiology, and we could see it everywhere in the Course - across LIVE cases, case-based discussions, Learning Sessions and practical workshops. The focus remained firmly on what matters most in the cathlab: decision-making, preparation, and learning from real-world complexity. At the same time, it was encouraging to see the continued growth of late-breaking science and research, something which is increasingly a focus for us as Course Directors. I was particularly pleased to see the enthusiasm around two new additions shaped directly by educational needs expressed by our community: the Calcium Skills Lab and the dedicated CTO track. The level of engagement throughout both was remarkable. Thank you to the 12,147 participants and presenters who made these exchanges possible. This past week, Paris – and #EuroPCR - once again became the focal point for the interventional cardiology community from across the globe.   What stood out for me was the depth of discussion around increasingly complex valvular interventions: lifetime management, multimodality imaging, complication prevention, and the constant refinement of our decision-making. Across sessions, LIVE cases and conversations, there was a clear sense of momentum.   The big increase in simultaneous publications, as well as the quality of the late-breakers and hotlines, indicate a growing scientific presence, as an accompaniment to the highly educational programme.   Thank you to all colleagues who brought their experience, questions and critical thinking to these four days, and to all those involved in the programme for their many hours of preparation.
