STEPS IN FILING SSS DISABILITY 1. Make SURE na may active disbursement bank account na tayo sa SSS before filing your claim. 2. GO TO SSS for the form (Complete set) or DOWNLOAD the SSS DISABILITY FORM WITH SSS MEDICAL CERTIFICATE.
