Critical illness always puts the brain in the line of fire and it has been suggested that a brain-oriented approach should be a unifying concept in the management of all critically ill patients.

Care protocols that aim to optimize cerebral physiology are routinely applied during the management of acute brain injury, and similar approaches are likely to be relevant more widely.

Whatever the cause of admission to the ICU, there is increasing awareness of the importance of the adverse effects of critical illness and its treatment on both short- and long-term brain function.

It may relate to critical illness-related inflammatory changes, neurotransmitter imbalances, or failure of adequate energy substrate delivery, as well as to the applied treatments, pharmacological neurotoxicity, or the hostile ICU environment.

The pathophysiology is complex and incompletely understood.

Life-threatening systemic insults or diseases often affect the brain. In critically ill patients, acute brain dysfunction manifests in several ways including reduced consciousness, coma, or delirium.

ICU Rehabilitation = Only through partnership with patients, families & MDT can we understand expectations, needs & goals to optimise lives & outcomes.
