@sandi89701936 @CBCQueensPark @gmacofglebe I will confirm that we MD's pay out of pocket to give vaccines. And to do most other procedures as well. We have to pay for all our equipment and supplies and get paid close to zero for performing these important procedures.

@CaffeineScene @cspotweet Good question. There are things I'm comfortable diagnosing & not. The level of comfort will vary between pharmacists & there expertise in different therapeutic areas. Pharmacists in general are very risk averse.

@DrNoelGrisdale @vettergreenart The minor complaint could be more serious & then we'd refer. We do know what we don't know. Most pharmacists are risk averse. Many chefs are ok if there is communication, and we must according to our SOP. There are also times when there are no other cooks than a pharmacist.

@psychrx @vettergreenart I appreciate that perspective. I’m concerned it’s a slippery slope. “Minor complaints” can morph into the unusual/complex & more serious. Outcomes best when a pt has a team practicing longitudinal comprehensive care. There’s a reason for the saying “too many chefs spoil the soup”

@sandi89701936 @mverbora @CBCQueensPark @gmacofglebe Every $1 spent on pharmacist services the system saves $2.50. The dispensing fee paid by the Ontario government is $6.83. Over $2 billion has been cut from pharmacist services. I was laid off & had to move to Alberta & live with my mom. Sorry if I'm not crying for physicians.

@sandi89701936 @mverbora @CBCQueensPark @gmacofglebe Pharmacies get paid $75 once per year for a thorough med review. With prep and documentation a 20 min call actually takes 1 hr. Most pharmacists do that. If it is a 5 to 10 min call then they aren't doing it ethically.

The $20 is for non-publically funded shots and is billed to the patient. Pharmacies have higher overhead than physician offices. The dispensing fee is $6.83. The cost to the pharmacy is $14

I have to pay for the syringe, alcohol swabs, needles, diluting agents, fridge with temperature alarms, monthly fee for sharps container, bandaids and nurse. I lose money on every injection. Pharmacy charges $20 private per shot

@CBCQueensPark McGunity cut $1.5 billion from pharmacies. I was laid off and had to go to Alberta to live with my mother! Ford has cut millions as well. No raises for pharmacy services. We worked during the pandemic when clinics closed. Pardon me if I don't feel bad for docs.

@CBCQueensPark It's misleading to compare to pharmacists medication reviews. A dispensing fee paid by the government is $6.83. Pharmacies have higher overhead than your average family physician so the average cost of filling a prescription is $14. Do the math on that one.

@LastStopJimbo @DrNoelGrisdale Pharmacists are trained in diagnosis, with an emphasis on minor ailments. We know when to refer. We don't want to be mini physicians. We can help primary care by taking less complex cases. Last I checked more physicians are sued than pharmacists.

@DrNoelGrisdale If anyone is foolish enough to get a Pharmacist's diagnosis, I would recommend everyone record the interaction for future legal use. There are dozens of "Background video recorder" apps for a phone. This allows you to record front or rear camera even when the screen is locked.

The HUGE difference here is that MD owned private clinics are not operating in a massive conflict of interest whereas the SDM pharmacy care clinics will be….and the “patient” will be harmed either via their wallet or via inappropriate care or likely both

Minister LaGrange reiterates Alberta is not funding pharmacy-led clinics. Adds it is a complex model, says it is a private clinic that is publicly funded, not unlike how many doctors operate their own businesses.

@DrNoelGrisdale Finally (I think!), there us a Uof A study that showed pharmacists diagnose, treat and monitor hypertension not only cheaper but BETTER than physicians. As per your harm quote, pharmacists no more harmful than physicians can be.

@DrNoelGrisdale We're not trying to replace physicians (although to some extent we did at the beginning of COVID when many physician clinics and ERs closed and we stayed open). We can take on the more minor ailments so physicians can handle more complex cases.

@DrNoelGrisdale We know what we don't know. We're health care professionals with a minimum 6 years university. We know when to refer to a physician, just as I hope you know when to refer to a specialist. We have no more conflict of interest than professionals such as physicians and dentists. 2/

@DrNoelGrisdale Patients at pharmacist clinics can take their prescription, if they get one, to any pharmacy. And frankly calling them clinics is just marketing, as pharmacists in your average pharmacy do this work anyways. BTW pharmacists including myself catch physician errors every day. 4/
