@KWebbNurse @Leicnut NG is best for short term feeding, to get someone over an acute episode of poor oral intake. Longer than this and if poor oral intake likely to be prolonged gastrostomy maybe considered. Gastrostomy may not be suitable eg GI anatomy is skewed and surgery may not be appropriate.

@Leicnut @KWebbNurse There's a risk in everything really, that's why it's part of our clinical judgement to weigh up the risks and benefits of oral, enteral or parenteral nutrition on a case by case basis. No two patients are alike.

@Leicnut @KWebbNurse Absolutely an essential part of nutrition support when a patient is unable to meet nutritional requirements. Just in my experience those with poor cognition need to be closely monitored. Had an LD pt with mittens, bridle and 24 hrs 1:1 carers and she still managed to get an NG.

@KWebbNurse @Leicnut Yes I have to admit I'm not a fan as also as a graduate had a patient dislodge there NG and filled there lungs with feed so only recommend when needs must and am very conscious nursing staff closely monitor tube position and pt.

@rd_poole @Leicnut Great stuff! I’m very dubious about NG tubes being used for feeding purposes though? Such high risk of misplacement and mortality.

@rd_poole Reminds me of being phoned by ITU about a pt who’d infarcted most of their bowel and been put on Liverpool Care Pathway (long time ago). I stopped LCP, started PN and subsequently HPN. They left hospital and have had a fulfilled and active life since. Nutrition saves lives.

@Leicnut within a couple of days pt sitting up in bed recovering well. The pt was in a heavier body so I think the fact that she was malnourished was overlooked. It was at that very moment I realised I had chosen the right career and how important nutrition is in the clinical setting. 3

@Leicnut she had diarrhoea and severe damage to her pressure areas. I'd just graduated and knew there was something I was missing so spoke with a HPB dietitian. We started the pt on a semi elemental feed and asked for a faecal elastase test. Turns out our suspicions were correct PEI, (2)
