🚨 New HSSIB Report: Insulin Safety & Mental Health in the Community
In our video, Nicola Milne & Professor Sam Seidu reflect on this important report, which the PCDO Society contributed to as a stakeholder.
The report highlights serious safety risks for people with diabetes who also experience mental health problems, particularly around insulin self-administration:
• Missed or unsafe insulin use (omission or overdose)
• Poor coordination between diabetes and mental health services
• High-risk patients falling through gaps in care
• Limited access to specialist support and technology
💡 For primary care, the message is clear:
• Be proactive in identifying risk, don’t assume others are managing it, and take a coordinating role when care is fragmented.
⚠️ Content note:
This report considers the care of people experiencing mental health problems and includes discussion about eating disorders, self-harm, suicide and death. Some readers may find the contents distressing. Information about mental health support can be found via the NHS and charities such as Beat Eating disorders.

Updated NG28 NICE guideline on management of type 2 diabetes in adults
We commend the moving of SGLT2 inhibitors and GLP-1 receptor agonists further up the decision trees; the focus on cardiovascular and renal disease, obesity, early-onset type 2 diabetes and frailty as special groups; the practicality regarding SGLT2 inhibitor safety around sick day rules, diabetic ketoacidosis and ketogenic diets.
We acknowledge that there will be some challenges around implementation, especially with regard to drug availability and financial strain around GLP-1 receptor agonists, and regarding workload and capacity in primary care.

Health should not depend on postcode, income or background, but too often it does.
Widening health inequalities are damaging patients’ lives and placing an unsustainable burden on the NHS and its staff. Doctors are left treating the fallout of problems they can’t fix alone, often feeling powerless.

This has been the fundamental switch in policy as led by @NHSE_WTE @NHSEngland over the last decade
Has there been any evaluation that this has improved things in primary care (access, outcomes, safety issues) BEFORE the next version of the workforce plan?
Or do the hubris of a few continue the health system on a path of “more non-doctors” in primary care?
Or is it about friendships and the need to maintain those?
That’s how policy should work- evaluation based and change or continuance based on data
Then again..I am no workforce expert.
Just a face on an apron.
