@WeinrebIlan @ENTpathology Good point. I dont know if there is a clear cutoff regarding frequency (anyone?). From my understanding non-canonical means exception from the rule or typical findings. So, I have seen a few („recurrent“) identical „non-canonical“ (other-than-V600E) BRAF mutations.

Our cohort showed a high prevalence of MYB::NFIB gene fusion, which can also help to confirm diagnosis in cases with unusual morphology (such as the so-called metatypical AdCC).

In concordance with our previous work, that IHC-PSMA negative areas are associated with decreased visibility on 68Ga-PSMA-11 PET scans, the IHC-ERG+ cases showed decreased visibility on corresponding PET scans.

In our cohorts we identified a frequent molecular subgroup of primary prostate cancer, ERG+ by immunohistochemistry (IHC), which does show an increased heterogeneity of IHC-PSMA expression by IHC, and above all more IHC-PSMA negative areas.

@Ruthra3596 Well, with regard to the purely biphasic differentiation thats also a good differential diagnosis. They recurrently harbor ctnnb1 mutations, usually show more stroma and are mainly observed in the major salivary glands.

One can discuss morphological and molecular overlap with highly unusual epithelial-myoepithelial carcinoma of the sinonasal tract, but there are some arguments to favor a spectrum of LGSNAC. More to come soon!
