Listing my 2025 publications. Most happened to be single-author papers this year. Many topics but single theme: "Misaligned incentives drift a reasonable clinician away from patient's interest" One/two takeaways per paper in thread below.. Some behind paywall -- DM for full-text.

Agree. Hard to uproot because they're embedded in design of academic incentive structure. We argued earlier this year that individual transparency is not adequate; need governance responses to realign incentives and institutional roles. Not easy! nature.com/articles/s4157…

Good read. Problem IMO is “nonfinancial” COI are inevitable in academics, b/c the system rewards conceptual ownership, visibility, pos results. These incentives can’t be disclosed away. Solution = redefining academic success to value methods & reproducibility alongside outcomes

Headlines reported 12 former FDA commissioners oppose the Trump administration's higher standards for vaccines, giving the impression that the new FDA bosses are deviating from established science. But SEC filings and OpenPayments show a swampy revolving door: Scott Gottlieb - $2.1 million from Pfizer since 2020 according to SEC filings. He was often the highest paid member of the board, where he is the lead advisor on ethics and regulatory compliance. Stephen Ostroff - $752,310 from Pfizer in consulting fees since 2020, according to OpenPayments. Mark McClellan - $3.3 million from Johnson & Johnson since 2013 according to SEC filings. He also consults Blackstone Sciences, which is led by Pfizer's former longtime top scientist whose first big investment was $750 million into Moderna's flu vaccines. Margaret Hamburg - $2.8 million from Alnylam Pharmaceuticals, which markets RNA interference (RNAi) technology Just two signatories lacked pharma ties. They are David Kessler, who executed Biden's COVID vaccine policy, and Jane Henney, who authored a prominent CDC-underwritten study defending CDC's COVID vaccine safety surveillance.

12 former FDA commissioners from both parties just went on the record calling Vinay Prasad’s vaccine framework a threat to public health. They bring more than 3 decades of leadership at the agency, and it is hard to think of a more forceful warning. 🧵 nejm.org/doi/full/10.10…

And finally in @CMAJ of @CMA_Docs Permission to Exist: Takeaway: A personal piece on earning credibility under scrutiny as an IMG in Canada. The conclusion is simple: stop auditioning for belonging. Hold your space; you don't need permission to exist! pubmed.ncbi.nlm.nih.gov/41285459/

@NatRevClinOncol Non-financial conflicts of interest: Takeaway: Most COI in medicine affecting judgment are not financial. They're insidious, invisible & affect what's believed, published, and offered to pts. Problems're structural, solutions're overdue! pubmed.ncbi.nlm.nih.gov/40903524/
