@slahri7 I like the mnemonic HOME: Pt gets discharged HOME H- health advocacy : teach Pt about their condition O- outpatient: arrange for outpatient/clinic f/u M- medications: arrange TTO medications etc E- explain: when to return to hospital (not better/worse/danger signs etc)

Does anyone have a checklist prior to discharge eg( patient understands condition/ meds ) #patientsafety
