New specialist paramedics are being trained to care for Londoners as demand for mental health support surges. In a first for London, these clinicians will be deployed by car to care for patients with the most complex mental health conditions, such as those at risk of suicide.

This one might be labelled “Airway management in trauma” but what we mean is “This episode might change everything we know about airway management in trauma”. Give it a listen and let us know what you think!

We recognise some members have questions about the recent survey on advanced practice roles. The survey is exploratory, and we’re monitoring developments to ensure advanced paramedic practice is understood and supported. Read the full statement ➡️ bit.ly/4ceFSpz

@aled_fellowes I dont think paramedics can do same as GPs. GPs manage complexity really well. And paramedics should not replace drs. But i do think we have a useful scope from base training that can be deployed appropriatley across primary, urg and emerg care to compliment the team.

@simontutt88 I don’t think some are even doctors tbh, seems to suspect. Ultimately, there’s nothing different a GP can do most of the time to what a paramedic can do. I’m all for keeping GP’s free for GP stuff and other HCPs dealing with stuff appropriate for their training

More misinformation being spread that advanced practice is not regulated. Well thats not true it is regulated if you are a hcpc registered profession. hcpc-uk.org/news-and-event…

@simontutt88 The logical extension is that advanced practice has to have a process of mandatory regulated training and qualifications in the particular specialty and it is then not possible to change specialty unless further training is done. At the moment such a process is totally absent.

@zaynhasn @aled_fellowes I dont agree paramedics are working at the same level as a gp at all even adv paras. GPs manage complexity and wide ranging primary care so well. But i do feel paras have an area we are good at and we add value as additions and value in urgent abd emerg care. Not as replacement

@aled_fellowes @simontutt88 And if you ever sat in one of my callback to the crews regarding why ive disagreed with them on a conveyance issue when they think somebody can be managed in the community you could see why. But even then we have to simplify our explanations to what is pretty complex, medically

@cbehaviourist @DrOKaneAgain I dont think we do. We are doing exactly what paramedics are trained to do, what the hcpc say is our job role and what the professional body representing paramedics in the uk say is our role. I dont think people realise what we are trained in.

@DrOKaneAgain @simontutt88 We now live in a dangerous world where Paramedics are doing GP and Doctor's work. They have an inflated view of there skills. I speak as a retired RN Medic.

@Montyburnout123 Yes which is why im a thorough supporter of people who working in advanced practice having credentials like RCEM, FICM, PHEM, PC Roadmap etc. I also feel the base profession experience should be in the speciality e.g a stroke nurse of 10 years training as a stroke AP.

@simontutt88 But I presume you’d agree there is a fundamental difference between a regulated specialty with an approved and regulated training scheme vs. hoping someone works within their scope of practice?

@Montyburnout123 But the professional holds themselves accountable and shouldnt practice. Id like to think that drs at the point of cct are self aware enough they wouldnt practice in another speciality without further training not cause there regulator is only thing stopping them doing so.

@simontutt88 What you’ve described is an employer process. This is different to the function of a regulator. Where a doctor has a CCT the regulator registers them as competent in that specialty, have completed an approved training process. This is the fundamental difference.

@Montyburnout123 We would insure we are re trained in a new field. Same as when i recruit a qualified ACP in a diff field they become a trainee again. Its down the professional knowing what there competence and scope is and being accountable.

@simontutt88 If you were to move from working as an ACP in emergency care to, for example, running heart failure clinics, how would the HCPC assure themselves you had the necessary training, skills and qualifications to do so?

@VincentVanGrump @Amb_Jon @DrOKaneAgain Where as paramedics go to uni for 3 years and do nothing, then see no pts. Do no exams and my 25 places para course with over 1000 applicants isnt selective at all. 🙄 you lot havent got a clue.

@Amb_Jon @simontutt88 @DrOKaneAgain Hi Jon. A GP must complete year of after of clinical and practical exams every year through 5 years of med school (a highly selective process just to enter) and then take a nationally standardised finals exam to demonstrate competence. That's just obtain an internationally..

@WiseOwl1730570 @RCEMPolicyVP @DrOKaneAgain Doctors move from f1 through to consultant based upon experience and post grad training. The paramedic profession is no different in terms of paramedics moving from initial qualified to experienced advanced levels of practice within the remit of paramedicine.

@DrOKaneAgain @LewisMcColl_ Compete for a 25 place with 1000 applicants course to get onto a paramedic degree, see hundreds of pre hospital patients, do post grad exams in paramedicine and then you can comment on how the paramedic profession practices.

@LewisMcColl_ Happy for that to happen. Get the A-levels. Get into Medical School. Spend 5 or 6 years doing intense competitive exams. Complete two foundation years. Then do several years of higher professional training & pass the competitive exams. Fine by me. But no cos-playing amateurs.
