How would you manage her follow up? Like…only clinical + blood test? Imaging? Frequency?

@nihardesai89 Great thread! just a little typo here for L-HES :) I would have also added a quick one to look for a CD3-CD4+ T cell population for L-HES screening. They can be tiny (<1%) but quite characteristic, and for steroid sparing we could use other DMARDs if biologics not available!

HES could be d/t a myeloid neoplasm (M-HES), clonal lymphocytes (T-HES) or even be familial. But most often our investigations don't reveal anything and we are forced to call it "idiopathic" HES 😭 #MedTwitter
