Anastomosing hemangioma it is. 😊 low power lobulated architecture and often associated with a medium-caliber vessel, and may show focal extension into adjacent adipose tissue ( 😱). The hallmark is anastomosing sinusoidal capillary-sized vessels lined by occasional hobnail endothelial cells. Cytologic atypia is minimal, and mitoses are absent or rare, supporting its benign nature. Additional clues: eosinophilic hyaline globules and extramedullary hematopoiesis can be seen. Got a call from a VERY angry clinician today asking me why I didn’t write “clinical correlation required” in the pathology report. “You omitted that critical phrase, so I sent the patient home, sat there drinking my coffee and didn’t correlate clinically. Sheesh!”

@howardm19 @KaminskiMed @SansanoValero Yes to both but there is no guarantee of a correct answer or correct assignment of etiology no matter how much you integrate. Most diagnoses are judgement calls. The field is crippled by the absence of a gold standard and the emphasis on etiology

Then you know 1) for great majority patients presented, substantive management decisions are made on basis of integration of clinical, historical, & imaging features 2) that integration never "overrules" pathologic findings in (minority of) cases when available.

Agree! 🤣🤣 Also if one asks how we know that the MDD diagnosis is correct or why it is the gold standard, the answer is...because we said so! 🤣🤣🤣 The MDD is the judge of its own accuracy so it can never be wrong! What a cool system!

@smlungpathguy Actually MDD is like - it’s obviously a 4 legged mammal , let’s make an effort to rule out it’s not a cat, or, more commonly, wait a few months, retake the photo to see if it does not turn into a rabbit

@smlungpathguy I never use that phrase. Ever. We pathologists are physicians and take ALL data into account to render our report. Conversations in complex cases with our collegues submitting the case are often done to provide the best care for our patients
