As a now progressive who came from a rural, conservative area I would recommend anyone who can’t understand why Trump won read this. It might be hard to swallow, but until we reconcile with this, dems will lose & progressive causes will never be realized. nytimes.com/2024/11/06/opi…

We started fellowship in 2020, when our ICUs were filled with intubated patients due to C*VID. Many ups and downs but couldn’t have chosen a better group of humans to grow with. Officially pulm crit care doctors!

@ajai_dandekar I am one of the lucky people who’ve had her as an attending during medical school, residency, and fellowship. A model of both brilliance and kindness, and frankly just fun to be around. Sad day for @uwpccm but hopefully a happy one for her.

Coping with my fulminant #ATS2022 #ATSFOMO the only way I know how… Wishing I was in SF with all my friends, so I will be attending therapy sessions with my 2-wheeled friends for the next week.

@kriscrothers Oh no! Sorry to hear but glad the sx are mild. For me it was my wife and daughter who tested (+) 3 days before I was to fly out (they’re already recovered). Didn’t want to possibly spread to others so stayed home. Really hard to see all the happy posts and not be part of it 😭

@ParijatSen11 Really cool case! Agree completely with you that TBBx for infections is risk >>>> benefit and probably shouldn’t be routine. I’ve had several cases lately of DFA(-) / PCR(+) PJP in non HIV immune suppressed pts. (usually cancer or BMT).

With Pneumocystis, the conventional teaching is, BAL has > 90% yield in HIV patients (high fungal load) as opposed to other immunosuppressed patients, who need TBBx in addition. Case in point, this data is before the advent of Pneumo PCR which has higher sensitivity 👍🏽
