Our society appears to tolerate a grossly incompetent and unsafe healthcare provider, an ineffective and incestuous Medical Council all supervised by an insipidly toothless HIQA and overseen by a government department whose very existence is a contradiction in terms. How in the heck did we arrive here, dear electorate?

Wrong-site surgery is classified internationally as a “never event”. When basic safety systems work, these incidents should not happen. At the same time, a national audit of waiting list governance at CHI Crumlin found procedure coding mismatches between clinicians’ records and national datasets. Patient safety depends on reliable systems, accurate records and strong clinical governance. In 9% of cases, the procedure recorded in the national dataset did not match the clinician’s description. Examples identified by the auditors included: •Excision of accessory digit (foot) recorded as amputation of a complete digit of the hand •Valve repair recorded as non-invasive diagnostic test •Dental extraction recorded as comprehensive oral examination •TPVR procedure recorded as non-invasive diagnostic investigation These examples come directly from the audit findings. @CHI_Ireland @npsoIRL

Doctors accidentally operate on patient's wrong testicle during surgery at Irish hospital The Health Service Executive (#HSE) has described the “low” number of such incidents over the two-year period as “encouraging”. However, a patient advocacy group criticised this stance and said wrong-site surgery should never happen. “Four patients underwent operations on the wrong part of their body. For those four people and their families, there is nothing encouraging about it,” said Stephen McMahon of the Irish Patients Association. #WrongSiteSurgeries thejournal.ie/testicle-surge…

@TFIupdates Any chance you'd know why the 103X ghosted this morning? We already have to drive to the north end of Ashbourne to be sure of getting on. Now spent 40 minutes standing in the cold because the 07:05 simply didn't show.

@TFIupdates Thanks RS. Can you explain what happens to the logged incidents? Is there a point at which TFI takes action against @Buseireann for its ongoing failure to provide reliable commuter service to+from one of the largest commuter areas (Ashbourne/Ratoath) 28K+ people serving Dublin?

@TFIupdates No need to PM: 103X was on the schedule at 07:05 at Ballybin Road. It never came. My daughter stood until 07:40 when the next one came. At 07:54 it left Aldi stop: full. - She's late - No one at stops south of Aldi is getting to work - 103 doesn't go to her destination

We need to realize the architecture of this isn’t at all the chaotic mess it may seem. It’s deliberately constructed like this so that people fall between stools, get shoved from pillar to post and noone has to take ownership or responsibility for anything.

In seeking accountability, survivors are bounced between government departments, Tusla and the Charity Regulator each citing limited remit. Tusla can investigate but can’t enforce reform. The Regulator says safeguarding culture isn’t its role. The result? An accountability vacuum

BREAKING: MAGA lunatics target Renee Good’s 6-year-old child by flooding his school with harassment and threats — forcing them to switch to online-only classes. If you were wondering how low MAGA outrage culture can sink, here’s your answer: they’re terrorizing a 26-student elementary school because a grieving 6-year-old goes there. After Minnesota mother Renee Good was shot and killed by a federal immigration officer, profit-hungry right-wing media and callous MAGA influencers didn’t stop at smearing her. They went after her son’s school, too. Southside Family Charter School, a small, community-centered K-5 school where kids eat meals together, mentor one another, and take field trips, was forced to move classes online after a flood of threats poured in. Why? Because the New York Post and MAGA social media mobs decided the school’s social justice curriculum made it a political target. Let that sink in: a school teaching kids empathy and history is now being treated like a crime scene. The school’s “offense”? Teaching about George Floyd’s murder. Posting a sign reminding ICE that they need a judicial warrant to enter. Encouraging staff to follow the law and protect students. In MAGA world, that’s apparently enough to justify threats, harassment, and calls to defund a school full of children. Right-wing accounts went full conspiracy mode, claiming without evidence that Renee Good was a “domestic terrorist,” that the school “trained” her to fight federal agents, and that her death was somehow justified. One TikTok pushing this garbage racked up more than 100,000 views. And then came the feckless politicians. Republican Rep. Buddy Carter of Georgia, safely hundreds of miles away, demanded that all federal funding be stripped from the school, calling it “anti-American.” Because nothing screams patriotism like punishing traumatized kids and educators for not worshipping ICE. This is the MAGA playbook in plain sight: Dehumanize the victim. Smear the community. Target the children. School leaders, who are trying to grieve while protecting their students, shut down social media and pulled their online presence just to stop the threats. Still, they emphasized what MAGA rage can’t erase — the fact that the community has rallied around them. This isn’t about safety. It’s not about education. It’s not about the law. It’s about sending a message: If you question authority, teach history honestly, or dare to humanize immigrants, MAGA will come for you—even if you’re a tiny school full of children mourning their friend’s mom. They silenced Renee Good with bullets; now they’re trying to silence her son’s classroom with false words. Please like and share to spread the disgust!

Greenhouse gas emissions intensity in the EU decreased by 34% from 2013 to 2024. The largest reductions were recorded in Estonia (-64%), Ireland (-50%) and Finland (-44%). This is a major achievement for Ireland, one of the top three biggest reductions in the EU.

@grok @DarraghOBrienTD Ah so greenhouse gas emissions intensity is measured as a % of GDP. Irelands actual reduction in absolute terms was only about 10-15%. Thanks.
