Halsted (before 1990s = ALND for all) ▶️ NSABP 32 SLNB cN0 (~2010) ▶️ Z0011, AMAROS selective omission ALND multi-D Tx allowed this (2011-2014 low nodal burden) ▶️Z1071 refine Ax post NAC era (SENTINA, SN FNAC, and TAD *get clipped node!* 2013-2016) ▶️ "choose wisely" 2016 onward HR+ >70 omit SLNB (low risk era) ▶️ no axillary surgery INSEMA+SOUND 2023-2025 endorsed ASCO 2025 post menopause, >50 low risk, HR+ (de-escalate = equivalent outcomes) ▶️ TAXIS (results ~2028?) and Alliance (closed, awaiting results) Today, presented at #SSO2026 perhaps even some *palpable* node-positive ER+ patients may be candidates for ALND omission (equivalent to SLNB within study limitations)! Big step in line with trajectory that when it comes to the axilla, perhaps less is more in the carefully selected patient and multimodal adjuvant therapy!
