What if the benefits of exclusive enteral nutrition for #Crohnsdisease are related to its impact on immune cells?
🆕 findings from @Bernie1234White, @PaedsRH, @KGerasimidis, @s_milling & colleagues 👇
📌 8 weeks of formula-based diet (no solid foods) impacts host immune cell profile in 9 children with #Crohnsdisease
These changes:
🔸were conserved in patients who achieved remission
🔸persisted in children who achieved remission and followed the nutrition formula consistently
💡What is new?
1⃣ EEN led to:
🔸⬇ TEMRA T cells and natural killer cells (associated with chronic inflammatory conditions)
🔸⬆ Central and effector memory CD8+ T cells (confer protection or further amplify the response at inflamed tissues)
2⃣ The impact of EEN on immune cells was independent of concomitant medication use (Azathioprine, Mesalamine, Budesonide, Prednisolone)
3⃣ Enteral nutrition led to changes in immune cells that persisted in children who achieved clinical remission or followed the nutrition formula correctly and consistently
🧑🔬 The authors hypothesized:
These findings are a consequence of the action of nutrition formula in decreasing the diversity of gut microbiota ➡ ⬇ recruitment of immune cells from the blood into the intestine.
🎯Now we are closer to understanding that the benefits of replacing all food and drinks with specialized liquid nutrition formulas for inducing Crohn's disease remission in children is partially related to changes in the immune system.
We look forward to upcoming research looking at the role of counterintuitive effects of EEN on gut microbiota in decreasing the activated immune response seen in Crohn's disease.
