The vast majority of commercial “value based” care is bullshit. There’s only two ways to drive down in-plan-year cost (the only thing the payer cares about) — care avoidance or shifting to lower cost site of care. Nearly all the other clever models or things optimized for long-term outcomes (which the payer doesn’t actually care about) don’t work.

One thing I’ll never understand about healthcare startups is the instinct to take a ‘strategic’ route rather than an administrative / tactical approach. You don’t need a top-to-top meeting to get in network with a payer. Just fill out the paperwork. You don’t need a strategic relationship with quest / labcorp to integrate scheduling or get competitive pricing. Just work with an existing vendor. If things go well you can elevate to a strategic relationship over time. But you’ll never get off the starting line with the strategic approach.

I’m pretty excited about my new angel investment. Tech friend said he had to manually take his data on a CD from one doctor to another. He had this amazing insight to create a cloud-based solution so data can just flow EMR to EMR. Seems like a $100B company. Anyone interested?

I've thought a lot about what resonates on X and drives engagement. People appreciate when others share insights that are unique to them and authentic. So I started thinking about what is unique to me. It is a helpful exercise in reflecting on the cool things about my life but also a huge motivator to do more! We've got one life, why not make it unique? Here is the start of my list 1. working in VC through 3 pregnancies in 3 years 2. incubating a gene therapy manufacturing co in columbus, oh 3. Living in Boston, New York, and Chicago before CHOOSING to live in Columbus, Oh because that is where I see the biggest opportunity for me and my family 4. going mountain biking with my coworkers, falling a minimum of 10 times (including off a bridge into a creak) but still finishing the course. I am the definition of an X lurker - always reading, never posting but I am going to try and change that in 2025 starting with the things I think are unique to me. We shall see how it goes...
