Imagine if every passenger on an airplane had a direct line to the pilot. That’s what medicine has become. We have embraced frictionless communication and that not a good thing. With things like Epic Chat, every member of the healthcare team has immediate access to the physician. Now there’s a constant stream of minor questions. It’s discouraged people from thinking critically. “Just ask the doctor,” has replaced any sort of clinical reasoning. When I was a resident, we had an answering service. Anyone who wanted to reach the doctor needed to go through a third party. This provided triage and accountability. If we were getting called in the middle of the night for stool softener orders, there was a record of that. A little friction in communication is a good thing.

A practical ICU lesson: If you’ve been with someone for years but aren’t legally married (or otherwise recognized under your state’s laws), don’t assume you’ll automatically be able to make medical decisions for them if they become incapacitated. If you want a specific person to make healthcare decisions on your behalf, complete the appropriate healthcare power of attorney/healthcare agent paperwork before there’s a crisis. Once someone has lost decision-making capacity, it’s too late to create those documents.

Karmelo Anthony has been put in isolation aka Solitary confinement. Why? Because although black gang members may end up viewing him a hero, white gangs won't, not to mention the garden variety whackadoodles who only seeks the fame of killing him for their own notoriety. He is in county right now pending his prison transfer. I have no doubt they may need to put him in Solitary for his own protection in prison too, at least for a time. He shouldn't have murdered Austin at that track meet. #AustinMetcalf #Karmelo
