This is not a complicated #GUPath case. These prostate glands are "too many, too small, and too crowded" to be benign. They also "cross the street" (i.e. traverse the entire width of the bx core). Other soft signs of PCA which, imo, are very helpful are seen here: crystals & amorphous pink secretions. Nucleoli aren't as prominent as one would want them, but the amphophilic quality of the cytoplasm is another, often neglected clue for PCA. Dx. GS336 prostate carcinoma (#IHCPath is CK5/p63 showing lack of basal cells) Some cases are more subtle! Prostate cancer with treatment effects following radiotherapy exhibits single cells with large, eccentric nuclei and abundant pale or foamy cytoplasm. Identification can be challenging at low magnification but is distinctly highlighted by PIN4 staining. Grading is not applicable in this context, and prognosis is generally better compared to cases without treatment effects.

Entities that are 100% H&E diagnoses are the best! Pleomorphic liposarcomas feature some of the most bizarre cells in human neoplasia. This example has beautiful pleomorphic lipoblasts, all decked out with lipid droplets crisply indenting their nuclei.

This gastric body biopsy shows severe destructive gastropathy - the parietal cells are gone! It is too inflamed for autoimmune gastritis/pernicious anemia. Apoptotic bodies, usually scarce in the stomach, are indicated. This was a result of pembrolizumab immunotherapy.
