Regarding RT - modality probably not as important as dose, volume, and coverage.

Applaud the authors for being aggressive, but once again biology wins. Almost 50% of pts with synchronous liver metastases had surgery first without chemo???

@GrossbergLab Right now we tend to reserve it for patients who have completed peri-op chemo but had either high nodal burden or a R1 resection. More is more, right?

#gastriccancer. ECOG 0, 3cm antral gastric 🦀 (ERBB2-, MSS) EUS cT2N2. Pre-op FLOT4➡️distal gastrectomy & D2. Path ypT3N3b 19/39 LN+, LVI+, PNI-, negative margins & poor/no treatment response for poorly cohesive carcinoma. 😔WWYD? Finish FLOT? ECF/EOX? ChemoXRT now or later? IO?

@drymtn IF there is any role for chemoRT in gastric, it's neoadjuvant. I'm not sure you can reliably identify an at-risk target post-operatively. And the efficacy will be limited by relative hypoxia in the field.

@KrishanJethwa I treat inguinals for any anal canal involvement. This is a disease for which we maximize local control.
I might reconsider for patients at elevated risk for toxicity (e.g. obesity) if minimal borderline involvement and cN0.
