Looking for a version of the MoCA that can be administered over the phone and was reminded of the Impossible MoCA, which is always fun to give to anxious colleagues. "Just a standard test I give to all my patients".

For every study that helps us decide whether to start a new medication, there should be an equal & opposite study to help us decide when to stop a medication. With that new law of medicine, I present you this study on beta-blocker discontinuation after MI nejm.org/doi/full/10.10…

In my advanced CKD clinic, this is a common clinical encounter: Mr. Jones is 83 and has chronic kidney failure with a low eGFR. He’s not eligible for a transplant.

SPRINT showed that intensive lowering of systolic BP increased the risk of incident CKD and episodes of AKI. A study looked at legacy effect of intensive vs. standard systolic BP lowering on longer-term incidence of kidney failure. Rates of dialysis or transplantation were higher with intensive treatment, but differences were small bit.ly/JASN0459
