Uncrossable degenerated surgical valve with critical stenosis. No big deal - transeptal access with J-tip RF steerable versacross (sheathless in RFV). Send angled-glide wire using same transeptal system in LVOT, across the valve and snare in the aorta from TAVR access. Externalize, send now catheter retrograde from TAVR access across the AV over the veno-arterial rail, release the rail and now send stiff wire.
