I have not found any historical or epidemiological evidence that using urine as mouthwash caused a specific endemic disease in ancient Rome despite the theoretical infectious risks of the practice. It is important to note that 1. Urine is not sterile when it leaves the body. if the donor had a urinary tract infection, sexually transmitted infection (e.g., Neisseria gonorrhoeae),or systemic illness (e.g., leptospirosis), pathogens could be present. However, Romans typically preferred urine from "untainted” youths (Pliny’s wording), perhaps reflecting an intuitive effort to avoid diseased sources. 2. Ammonia content in stale urine (from urea breakdown) does have mild antibacterial and cleaning properties. While not a reliable disinfectant by modern standards, it may have reduced some bacterial loads, lowering the risk of transmission compared to using plain water. 3. No contemporary account describes outbreaks traced to urine mouthwash. Roman medical writers (Celsus, Galen) mention various oral rinses, but they do not connect urine rinsing to epidemics. The major infectious burdens in Rome like gastrointestinal diseases, malaria, tuberculosis were tied to water supply, crowding and sewage, not to this practice. 5. Laundering with urine (fulleries) which was commonly also, carried occupational risks however. Fullers were exposed to ammonia fumes and possibly zoonotic pathogens, but again, no distinct 'endemic disease' is historically attributed to that trade beyond chronic respiratory irritation.
