@jerrytheamboman @vc_medic We say access can be achieved with in 10 seconds. Humeral IO flow rates average 105 ml/min under pressure. Takes 3 seconds to get to the heart.

@vc_medic Access can be achieved in under 30 seconds but flow rate is reduced. Some advocate for Intravaenous acess after the IO (when the resus is running)

Please stop reaching for cannulas in cardiac arrests…. Go for the Intra osseous kit Access can be achieved in under 30 seconds for skilled users, & all drugs can be put through it. NB: don’t take blood samples, as it will often trash your analyser!

I have just finished another set of incredibly difficult weekend nights as the A&E consultant incharge. Some thoughts: The NHS no longer provides unscheduled care - we provide treatment. Care is with dignity - that's impossible for our patients in corridors. The model of acute care is not working or sustainable. We need a completely different approach - with long term investment in prevention and community care 111 need to change risk thresholds as far too many come to a&e when they don't need to. There needs to be alternative pathways to get care overnight for those who don't have accidents or emergencies. A&E shouldn't be anything and everything Staff are broke - supporting staff is just as vital as looking after patients This government have totally decimated our the NHS. They are the ones who should be apologising to our patients not our staff. Hundreds are dying each week because of lack of hospital capacity and corridor care in a&e, yet it's not on the news and no public uproar. As staff and patients we have accepted the unacceptable. Everyone needs to know the reality of what is happening - so that politicians offer a different solution and patients get the care they deserve. Sadly most of the public have no idea of what is genuinely going on. And finally, a post night fry up is the best antidote for the stresses of the weekend. Goodnight!

💣🚨 Why will City’s case be decided so long after Everton’s case? It’s easy, #MCFC has delayed the PL’s investigation with pure stalling tactics resulting in that the investigation that started in 2018 couldn’t really proceed before July 2021. And the below excellent summary by @mjshrimper doesn’t even include the latest claim that the head of the panel to try MCFC’s case wasn’t impartial. There has been a big MCFC propaganda push lately claiming that the case is “different” than Everton’s case, is “very complicated” and must be given time. ⚠️But it’s a fact that the investigation against MCFC has been more or less delayed over — three years — due to ridiculous claims about impartiality among the extremely experienced lawyers appointed to decide on which documents MCFC should hand over to the PL; whether certain information about the proceedings could be made public etc etc etc — leading to no less than FOUR known court rulings to date (there could be many more), which ultimately led to a High Court Judge (Lord Moulder) describing City’s delaying tactic and arguments with words like “surprising”, “remarkable”, “unconvincing”, “fanciful” (lawyerish for “you [Lord Pannick, who City pay more than KDB] should be ashamed of yourself”). See below summary. ✅ So why have MCFC resorted to these ridiculous stalling tactics? Doesn’t it make them look guilty? It is — very — simple, it’s the ONLY CHANCE they have. It is true that both UEFA and, it seems, the PL have gone after MCFC with very serious allegations — based on the Football Leaks documents, which comes across as utterly damning — but that could be hard to prove. But the PL didn’t charge MCFC on these accounts alone, it went after the club with the “Al Capone” play, ie the mafia boss who after years of investigation regarding numerous murders, could be brought down, on tax charges... The charges doesn’t seem to have been made before the PL had so much on MCFC, that the outcome seemed to be a given. ⚽️So what chance does MCFC have? Simply a change of landscape. If someone told me that the current PL Rule Book in the relevant sections have been written to have MCFC expelled — I wouldn’t protest. The PL doesn’t have to prove anything beyond reasonable doubt, the lowest possible evidence standard applies instead. If the PL can show an email that suggests something — it’s up to MCFC to prove that they are “innocent”. Nothing is time barred as a rule. Etc etc etc. If the PL implement a 10 year statute of limitations MCFC escapes then oldest charges that seems to be the easiest to prove. Clear pay day loans from the owner, off the books payments to Mancini etc. Everything that was time barred in the UEFA cases. This combined with refusal to hand over documents they are obliged to share with the PL — over a long period of time — should alone be enough for being relegated. Anyway, both Tottenham and CFC is subject to very old incidents — could a majority be had? We also know that Abu Dahbi is described to be heavily involved in the Qatar Gate drama, regarding several members of the EU parliament being bribed. A lot can happen in the coming 2 years. ⚠️Why is this important to point out? City deserves a fair hearing even if the evidence against them comes across as utterly damming. But everyone should be aware of how MCFC will do what it said it would do, spend enormous resources to prevent such hearing from taking place.
