20 years ago @claassen_jan and I co-authored a systematic review of the relative benefits pentobarbital, midazolam and propofol for refractory status (RSE). They all worked about the same. This @NeurologyToday article highlights the clinical impact of a recent update on how we treat RSE. Upshot: 1. Ketamine has joined the fray, joining midazolam (most common), propofol, and barbiturates (pento and thiopental) as the most common 1st line continuous IV therapies for RSE 2. Phenytoin (not levetiracetam) is still the most commonly used 3rd line agent after 1st line benzos 3. Effectiveness of any cIV-Anesthetic therapy is much worse in acute symptomatic as opposed to epilepsy-related SE. My take is that the major unexamined issue is TIME TO TREATMENT. Let’s study this!
