50 regular coffee drinkers had their water intake replaced with coffee for 3 consecutive days. Researchers tracked total body water, urine volume, blood chemistry, and electrolyte balance. No meaningful difference in hydration status. That finding came from a 2014 University of Birmingham study published in PLOS ONE. A separate 2016 trial in the American Journal of Clinical Nutrition then tested 13 beverages against plain water. Coffee, hot tea, iced tea, cola, beer, orange juice, sparkling water, and sports drinks all produced the same urine output as plain water over 4 hours. The only things that kept more fluid in your body than water were milk and oral rehydration salts, the kind of packets used to treat severe dehydration. Caffeine is a diuretic. It signals the kidneys to hold on to less water, so more ends up in your urine instead. But coffee is more than 95% water, and at 1-4 cups per day the fluid you consume far outweighs the small amount caffeine redirects. Research shows the diuretic effect only kicks in above roughly 250-300mg of caffeine (around 3 cups of coffee), mainly in people who haven't had any in days. Your body adapts to regular caffeine within a few days, and the response shrinks. The FDA puts the safe daily ceiling at 400mg, roughly 4-5 standard cups. The patients coming in dehydrated are doing something different. Black tea runs 40-70mg of caffeine per cup, so 2 liters a day is roughly 8-9 cups and somewhere between 320-580mg total. Coffee would push that number much higher. Above roughly 500mg, even regular drinkers who have been drinking coffee for years start to see a meaningful rise in urine output. With nothing else to drink in the day, the deficit adds up. At 2 cups of coffee per day, the research says drink the coffee. At 2 liters of black tea with nothing else, the doctor is right.
