【Selected Papers 論文紹介】 『特発性炎症性筋疾患における患者報告アウトカムの性差：COVADデータベースのサブ解析』 吉田　晃 特発性炎症性筋疾患（筋炎）における患者報告アウトカム（Patient-reported outcome measures; PROM）における性差はこれまで十分に検討されていませんでした。2021年に世界中で実施された「自己免疫疾患患者に対する新型コロナワクチン接種に関する研究（COVID-19 Vaccination in Autoimmune Disease; COVAD study）」では、新型コロナウイルスワクチンや疾患活動性に関する情報のみならず、身体機能、痛み、疲労・倦怠感を含む様々なPROMが合わせて収集されました。本研究ではCOVADデータベースより筋炎患者さんのデータを抽出し、PROMの性差を検討しました。 1197名の筋炎患者さんからの回答が解析対象となりました。回答者の70%が女性であり、女性の筋炎患者さんは他の膠原病を2つ以上合併する頻度が高く（女性: 11.4% vs. 男性: 2.8%; P < 0.001）、疲労・倦怠感のVisual analogue scale（VAS）スコアが有意に高値でした（5 [3–7] vs. 4 [2–6] , median [IQR]; P < 0.001）。多変量解析により、女性、高所得国での生活、他の膠原病に伴う筋炎、2つの以上の他の膠原病の合併が、独立して疲労・倦怠感VASスコア高値と関連することが示されました。 本研究の結果から、疾患活動性のみならず患者さんの主観的な体験に注意を払うことの重要性、また他の膠原病を合併する女性の筋炎患者さんが有する疲労・倦怠感への介入の必要性が示唆されました。 doi.org/10.1093/mr/roa… #JCR #日本リウマチ学会 #ModernRheumatology #SelectedPapers #論文紹介
