The meaning of this panel as I have always said. He's looking away from her to look at himself only regarding his hedonistic desires when the very thing that pulled him out of the water is the person he had a genuine connection to. He never saw that, that's why he never saw her.

I haven't really known how to feel about Nayuta's death. But after today's chapter, I think it was the right move. A perfect consequence of Denji's flaws and actions.
He was told what to do to save her.
And he ignored it. He chose Chainsaw Man over her.
