In a Medscape Q&A, Patricia Ford discusses performing stem cell transplants without donor blood — an approach originally developed for patients who decline transfusions but now gaining broader clinical interest. She highlights advances that make these procedures safer, including careful blood management and supportive care strategies, noting the approach may have benefits beyond patient preference. Female reproductive cancers are a major driver of mortality in women aged 35–60, based on a large study spanning 20 countries and more than six decades. Breast and gynecologic cancers accounted for much of the excess mortality compared with men, and eliminating them could increase women’s lifespan by about 0.77 years. mdsc.pe/4bTODol

Use of psychotropic medications, particularly selective serotonin reuptake inhibitors (SSRIs) and anticonvulsants, was associated with bone loss at the spine and hip in men with lower BMI, a new cohort study showed. These associations persisted even after adjusting for factors such as socioeconomic status, lifestyle factors, and medications known to affect bone health. mdsc.pe/4lzeWDy

Colorectal cancer in the United States continues to move in two different directions — decreasing in older adults and rising in younger adults, especially in those aged 20-49 years, according to the latest statistics from the American Cancer Society. The ACS report, published online earlier this month, revealed that CRC incidence rates declined by 2.5% annually in adults age 65 years and older but increased by 3% annually in adults aged 20-49 between 2013 and 2022 — higher than earlier estimates of 1%-2% annual increases. The trends are “concerning” and a “stark reminder that we’re seeing a shifting epidemiology,” said Folasade (Fola) May, MD, PhD, MPhil, director of the gastroenterology quality improvement program at UCLA Health in Los Angeles, who wasn’t involved in the analysis. mdsc.pe/4bAN3X8

A growing debate is emerging as continuous glucose monitoring (CGM) gains traction. While A1C remains a simple, standardized measure of average glucose over time, CGM offers real-time insights into glucose variability, time in range, and hypoglycemia risk. Experts note that each has strengths and limitations: A1C is widely accessible and well-validated, whereas CGM requires devices, training, and ongoing data management. The takeaway? Many clinicians say it’s not about choosing one over the other — but using both together to get a more complete picture of glycemic control. mdsc.pe/4bXn2BR

A sugar solution placed in the mouth before a needle insertion likely curbs newborns’ pain, according to a new Cochrane review. The finding — combined with a 2023 Cochrane review and others — suggests that the longstanding practice now has enough evidence to be standardized across hospitals and used as a “gold standard” in clinical trials, the researchers said. It also adds to growing research on the effects of nonpharmacologic pain interventions for infants — and hints at lingering gaps in our understanding of how babies process pain. mdsc.pe/4bTt9Ic

New guidance on the use of GLP-1-based drugs in people with type 1 diabetes (T1D), due to be published later this year, will address the safety and optimal use of these medications which are currently used off-label in this patient population. Although the use is off-label, it has been increasing in recent years, particularly as an adjunct to insulin in people with T1D who also have obesity and/or cardiovascular disease. mdsc.pe/4rM8vPd

A major meta-analysis led by Jack Wilson, PhD, of the University of Sydney found little reliable evidence that cannabinoids effectively treat most psychiatric conditions or substance use disorders. The review showed no benefit for anorexia nervosa, anxiety, PTSD, or opioid use disorder, and insufficient data for ADHD, depression, OCD, and bipolar disorder. Some improvement was seen for cannabis withdrawal, Tourette syndrome, insomnia, and autism traits, but overall evidence quality was low. mdsc.pe/4sheT1N

US weekly magazine Newsweek and data platform Statista have published their annual ranking of the World’s Best Hospitals — regardless of specialty — for the eighth time. The Mayo Clinic in Rochester, Minnesota, ranked No. 1 for the fifth straight year. There were changes, however, just behind it: Toronto General Hospital and the Cleveland Clinic swapped places at No. 2 and No. 3. Stockholm’s Karolinska University Hospital also improved by one spot to No. 4, making it the highest-ranked hospital in Europe. mdsc.pe/4bwSAOv

Microbiome-modulating therapies, including prebiotics, probiotics, and postbiotics, are safe and associated with “clinically meaningful” lesion reductions in patients with acne, according to a recently published systematic review. Among these therapies, “probiotics currently have the strongest evidence base, while prebiotics and postbiotics are promising adjuncts,” the authors from the Department of Dermatology and Cutaneous Surgery, University of Miami, Miami, wrote in the review, published online in Dermatology and Therapy. mdsc.pe/41gxMpC
