Would we still be doing cord milking in extremely preterm infants if PREMOD2 did not allow for waiver of antenatal consent? Have a look at Dr. Barrington's take on our study of waiver vs no waiver eligibility? neonatalresearch.org/2026/03/27/neo…

Is this the way? Great paper to get your neonatal resuscitation team discussing. More trials will come. But maybe start thinking how you could do intact cord resuscitation in your system. @EBNEO

We are delighted to share that CORDMILK has completed enrollment with a total of 3447 babies. This is the largest cord milking vs early cord clamping trial of non-vigorous newborns conducted to date. Congrats to all of our investigators in India! @NIH @Ilcor_org @AAPneonatal

Dear #Neotwitter, ⁦@CardioNeo⁩ - are you a #NICU that practices LISA or INSURE in extremely preterm infants? Have you ever seen #pulmonary hemorrhage soon after #surfactant ? Does having a higher mean airway pressure after surfactant reduce risk of pulmonary hemorrhage?

@NeoAnup @Ilcor_org @EBNEO @neosatyan @AAPneonatal @henryleeneo Great work! You mention baseline differences in your discussion, and agree they are likely by chance. However - they still may have accounted for differences in the PO. E.g. antenatal steroid use (10% lower in low group) could certainly impact hypoxemia in first 5 minutes.

@drbretty @Atul_Monash @natetexsun Hi Brett. Not in the main paper (will be out next month) but in the substudy we will these data. I can’t say how often but enough times where it was low where I would argue that we should be looking at the baby and not the monitor.

#neotwitter poll: If you knew the heart rate of extremely preterm infants from the moment of birth and throughout delayed cord clamping, assuming the baby was breathing, when do you think the cord should be cut and the baby moved to the resuscitaire? (assume HR is not improving)

@drbretty @Atul_Monash @natetexsun Interesting question Brett since many of our doxie kids started out with low heart rates (using neobeat), but improved over the first 90 seconds. Agree with waiting!
