@WsSeaton @emmaspellmanOT So it is a primary care service and referrals come mainly from GPs, providing wraparound support in the community. Rather than a ward being made up of hospital beds, the patients’ own beds become part of a virtual ward, helping stop hospital admissions and relieving GP pressures

First week completed within the virtual wards, it is definitely a lot different to anything I have done before but very excited to see how the role develops 💚

@Leens_OT I saw the same on mine! Plus the extra burden on family become unofficial carers whilst ‘bridging the gap’ for care to start in a desperate attempt to get their loved ones out of hospital but having no firm date when care will start. It’s an awful situation 🙁

"In the 2021-22 report, Welsh NHS Confederation found that up to 1,500 medically fit patients could not leave hospital because of a lack of social care" - Reflecting on my placement in an acute hospital setting, I believe this figure to be a lot higher and it's heartbreaking.
